Sarcomatoid urothelial carcinoma
Diagnosis in short

Sarcomatoid urothelial carcinoma. H&E stain.
Subtypes (subtype of urothelial carcinoma)
LM DDx sarcoma, carcinosarcoma - esp. in women, other sarcomatoid carcinomas, collision tumour
IHC GATA3 +ve
Site urothelium - urinary bladder, ureter, renal pelvis

Prevalence very rare
Prognosis poor
Treatment resection if feasible

Sarcomatoid urothelial carcinoma (abbreviated sarcomatoid UC), also sarcomatoid urothelial cell carcinoma (abbreviated sarcomatoid UCC), is a rare variant of urothelial carcinoma.[1]

General

  • Rare[1] approximately 0.1% of bladder carcinomas - based on the SEER database in the time span 1973-2004.[2]
  • Aggressive >55% dead within one year of presentation.[1]

Gross

Microscopic

Features:

  1. Urothelial carcinoma (classic or recognizable variant).
  2. Sarcomatoid differentiation - typically (undifferentiated) spindle cells.

IHC

  • GATA3 +ve (16 +ve of 22 cases[5]).
