Cytomegalovirus colitis
Diagnosis in short

CMV colitis. H&E stain. (WC)

LM enlarged nucleus - classically in endothelial cells, +/-cytoplasmic inclusions
LM DDx infectious colitis
IHC CMV +ve
Site colon

Associated Dx +/-HIV infection
Clinical history +/-immunosuppression, +/-immune incompetent (e.g. transplant recipients)
Prevalence rare

Cytomegalovirus colitis is a type of colitis caused by cytomegalovirus.

General

  • Uncommon.
  • Immunosuppressed population at risk, e.g. transplant recipients, individuals with HIV.
  • Typically self-limited in immunocompetent patients.[1]

Microscopic

Features:

  • Enlarged nucleus - typically in endothelial cells.
    • Nucleus classically has peripheral clearing - described as "owl's eye".
    • Epithelial cells and lamina propria cells may also be affected.[2]
