This article deals with lung lobectomy grossing.

Introduction

Lobectomies are typically done for lung cancer.

It is an intermediate size operation between pneumonectomy (removal of the whole lung) and segmentectomy (removal of a segment of a lobe).[1]

Specimen opening

Protocol

Lobectomy specimen:

  • Overall specimen dimensions: ___ x ___ x ___ cm.
  • Bronchus: ___ cm.
  • Peribronchial lymph nodes: [number].
  • Pleura: [smooth/irregular/puckering - size ___ x ___ cm].

Tumour:

  • Location: [left/right, upper/middle/lower].
  • Dimensions: ___ x ___ x ___ cm.
  • Appearance: [solid/cystic].
  • Depth of invasion: ___ cm.

Distance (from tumour) to margins:

  • Bronchial: ___ cm.
  • Vascular: ___ cm.
  • Parenchymal: ___ cm.

  • Uninvolved parenchyma: [description].

Representative sections are submitted in cassettes as follows:

  • Bronchial and vascular margin - en face.
  • Parenchymal margin.
  • Tumour.

  • Uninvolved lung parenchyma.
