The general differential diagnosis for histologic colitis (infection, inflammatory, ischemia, iatrogenic) should be considered. 
Inflammatory activity is mild and patchy, as could be seen in (partially) treated IBD (ulcerative colitis or Crohn's disease) or untreated Crohn's disease. Clinical correlation is required.
