ALK translocation renal cell carcinoma is a proposed entity within the Vancouver modification of the 2004 WHO classification of renal neoplasia.[1]
General
- Extremely rare - 1 in 829 consecutively screened patients.[2]
- May be related to renal medullary carcinoma which is reported to have an ALK rearrangement - t(2;10)(p23;q22).[3]
- May respond to ALK inhibitors, e.g. Alectinib.[4]
Microscopic
Features:
- Mix of morphologies - including rhabdoid change, sarcomatoid change, "unclassified renal cell carcinoma", papillary renal cell carcinoma.
DDx:
IHC

- ALK +ve.
- PAX8 +ve.
- Vimentin +ve (diffuse).
- CK7 +ve (focal).
