Sarcomatoid urothelial carcinoma (abbreviated sarcomatoid UC), also sarcomatoid urothelial cell carcinoma (abbreviated sarcomatoid UCC), is a rare variant of urothelial carcinoma.[1]

General

  • Rare[1] approximately 0.1% of bladder carcinomas - based on the SEER database in the time span 1973-2004.[2]
  • Aggressive >55% dead within one year of presentation.[1]

Gross

Microscopic

Features:

  1. Urothelial carcinoma (classic or recognizable variant).
  2. Sarcomatoid differentiation - typically (undifferentiated) spindle cells.

DDx:

  • Sarcoma - esp. those arising from the retroperitoneum.

IHC

  • GATA3 +ve (16 +ve of 22 cases[5]).
