Stratified mucin-producing intraepithelial lesion of the cervix is a rare dysplastic lesion of the uterine cervix. It is considered to be a high-grade lesion with overlapping features of endocervical adenocarcinoma in situ and squamous intraepithelial lesion.[1]

It is abbreviated SMILE (Stratified Mucin-producing Intraepithelial LEsion).

Microscopic

  • Stratified epithelium with:
    • Nuclear atypia.
    • Cytoplasmic clearing or vacuoles in lesions - through-out.

Features:

  • Ki-67 high.
  • Keratin 14 -ve.
  • p63 +ve/-ve -- only basal if positive.
