Signet ring cell-like adenocarcinoma of the prostate
Signet ring cell-like adenocarcinoma of the prostate is a rare aggressive form of prostate cancer.
It is also known as signet ring cell carcinoma of the prostate and prostatic signet ring cell carcinoma.
The term "signet ring cell-like" is preferred over "signet ring cell", as per the WHO Blue Book of 2022, as the cells are often mucin negative.[1][2]
General
- Very rare - 9 cases in a series of 29,783 prostate cancer cases.[3]
- Criteria vary - percentage of SRCs required for Dx varies from 20% to 50%.[3]
Microscopic
Features:
- Signet ring cells.
- Large cytoplasmic vacuole, peripherical nucleus with a crescent shape.[1]
- +/-Conventional prostatic carcinoma.
DDx:
- Acinar adenocarcinoma - Gleason pattern 4 with very small glands.
- Metastatic signet ring cell carcinoma.

Stains
- Alcian blue-PAS stain +ve.
- PAS - 50% of cases +ve.[3]
- Alcian blue - 44% of cases +ve.[3]
Note:
- May be mucin stain -ve.
IHC
- PSAP +ve.[3]
- PSA +ve.
- CEA -ve.
Note:
- If prostate stains (PSAP, PSA, NKX3.1) are negative, a GI primary should be excluded. Clinical correlation may be required.
