Tips of an ellipse involved

The lesion is clear of the margins in the plane of section in slide A2-1. However, the lesion is present in the mid portion of one of the tips (slide A1-1); this suggests the lesion is marginally excised or may be at the margin.

Actinic granuloma of O'Brien

SKIN LESION, RIGHT ANTERIOR SHOULDER, EXCISION:
- SMALL NON-NECROTIZING GRANULOMA, SUPERFICIAL, SEE COMMENT.
- SOLAR ELASTOSIS, MILD.
- NEGATIVE FOR MICRO-ORGANISMS WITH SPECIAL STAINS (ZN, GMS, PASF).
- NEGATIVE FOR MALIGNANCY.

COMMENT:
The granuloma could represent an 'actinic granuloma O'Brien'.

The granuloma is NOT typical of sarcoid. No dermal scar is seen.
