Pulmonary apical cap, abbreviated CAP, is a rare benign lung pathology that is found at one or both of the apices.[1]

General

  • Possibly due to ischemia.[2]

Gross

  • Spiculated mass at the apex - 0.7 to 5.2 cm.[2]

Microscopic

Features:[2]

  • Fibroelastotic scar with mature collage and wavy/curled (accordion-like) elastic fibres.
  • Alveolar enlargement at lesion periphery ("scar emphysema").
