Stratified mucin-producing intraepithelial lesion of the cervix is a rare dysplastic lesion of the uterine cervix.
It is abbreviated SMILE (Stratified Mucin-producing Intraepithelial LEsion).
General
- Rare ~0.6% of cervix lesions in one series.[1]
- Often accompanied by cervical intraepithelial neoplasia and adenocarcinoma in situ.[2]
Microscopic
Features:[2]
- Stratified epithelium with:
- Nuclear atypia.
- Cytoplasmic clearing or vacuoles in lesions - through-out.
DDx:

IHC
Features:
- Ki-67 high.
- Keratin 14 -ve.
- p63 +ve/-ve -- only basal if positive.
